PHOTO CONSENT FORM
Authorize MG Velvet Pro and its legal representative to capture and use photographs and/or videos of my image for the following purposes:
• Advertising and marketing on social media, websites, and other digital or print media.
• Before-and-after portfolio for demonstration to future clients.
• Educational or promotional material related to MG Velvet Pro services.
I understand that:
1. These images may be edited or cropped as needed for the purposes mentioned.
2. I will not be compensated for the use of these images now or in the future.
3. I have the right to revoke this consent at any time in writing, understanding that the revocation will not affect the prior use of the images.
INFORMED CONSENT FORM FOR MICROPIGMENTATION (PMU)
POSSIBLE SIDE EFFECTS AND RISKS
Irritation and redness in the treated area.
Temporary swelling or sensitivity.
Risk of infection if aftercare instructions are not followed.
Uneven pigment fading depending on skin type and care.
The need for touch-ups to achieve optimal results.
Rare cases of allergic reactions.
PRE-APPOINTMENT INSTRUCTIONS:
2 Weeks Before:
• Avoid undergoing facial treatments.
48 Hours Before:
• Do not take anticoagulants (aspirin, ibuprofen, vitamin E) unless prescribed.
24 Hours Before:
• Refrain from consuming alcohol or caffeine (coffee, tea, energy drinks, soft drinks).
AFTERCARE INSTRUCTIONS
I have been informed that I must strictly follow the aftercare instructions to avoid infections and improve the longevity of the pigment. This includes:
✔ Not wetting the treated area for the first 24-48 hours.
✔ Avoiding sun exposure, swimming pools, saunas, and makeup on the area during healing.
✔ Not touching, scratching, or removing any scabs that may form.
✔ Using only the recommended products for hydration and protection.
LIABILITY WAIVER
I understand that micropigmentation is an artistic procedure and that results may vary depending on my skin type, aftercare, and other individual factors. MG Velvet Pro and its staff do not guarantee a specific result.
I acknowledge that:
This procedure is voluntary, and I take full responsibility for my decision to undergo it.
I will not hold MG Velvet Pro or its staff liable for any adverse reactions, uneven healing, or pigmentation changes.
I have received and understood all pre- and post-treatment instructions.
